Position description
This position is open to experienced nurses and new grad nurses.

Location: Baldwin 4, Internal Medicine Primary Care Resident Practice, Primary Care Rochester

Patient Care Type: We care for adults 18 years and older who live within our local community as well as Mayo Clinic employees and dependents. Our providers offer general health and wellness care, including preventive exams. They also help manage complex medical conditions, such as heart disease, emphysema and diabetes, as well as help manage multiple comorbidities and geriatric care.

Nursing Team Roles: The LPN prepares patient for provider office visits which includes: medication reconciliation, reviewing preventative services due, vital signs, immunization administration, and collecting pertinent health information. Manage nurse visit calendar and post visit cares which include: medication administration, immunizations, blood pressure checks, suture/staple removal, dressing/wound care, ear washes, specimen collections and other nursing procedures. Work collaboratively with registered nurses and providers to relay test results, assist with prescription discrepancies, and other health follow up via telephone and online services.
For new grad nurses: All newly graduated Licensed Practical Nurses with less than 12 months of practice experience, hired into a scheduled FTE LPN position on the Rochester campus of Mayo Clinic are automatically enrolled in the Nurse Residency Program. A separate application or enrollment process is not required. All nurses new to Mayo Clinic, regardless of experience, will receive an extensive orientation program.

The licensed practical nurse (LPN) provides nursing care to assigned patients under the direct supervision of the registered nurse (RN) or other assigned supervisor and accepts delegation from the RN/assigned supervisor in meeting the needs of the patient/family. The LPN collects data through observation and communicates information to assist the RN and/or provider in patient assessment and care planning. The LPN participates as a member of the health care team and accepts delegation from the RN/assigned supervisor and provider in meeting needs of the patient/family. The LPN delivers care in accordance with the patient care plan, policies and procedures of the organization and principles of relationship-based care. The LPN possesses excellent communication skills; is skillful in mentoring and instructing; and may participate on committees or projects, including quality improvement projects.

Qualifications
For new grad nurses: All newly graduated Registered Nurses with less than 12 months of practice experience, hired into a scheduled FTE Registered Nurse position are automatically enrolled in the Nurse Residency Program. A separate application or enrollment process is not required. All nurses new to Mayo Clinic, regardless of experience, will receive an extensive orientation program.

Graduate of a school of practical nursing or passed LPN Boards after a defined period in a professional nursing program (e.g., some states allow RN students to take the LPN board exam after completing one to two years in the RN program). If graduation did not occur within the last two years, one year of LPN experience working in an applicable setting is required.

Additional qualifications
Excellent communication skills (verbal and written). Experience working in a team environment. Strong keyboarding and computer application skills required, prior experience with electronic medical record systems preferred. Ability to work flexible hours. Ability to adapt to unpredictable situations within the work setting. Demonstrated leadership, effective communicator, and excellent critical thinking skills.

License or certification
Current LPN license by applicable state requirements.

Midwest: Maintains current Basic Life Support for Health Care Providers from one of the following programs: American Heart Association or American Red Cross.

Additional specialty certification/training as required by the work area.

Compensation Detail
Education, experience and tenure may be considered along with internal equity when job offers are extended. The minimum pay rate is $21.80 per hour.

Will be expected to physically float to other Primary Care sites as needed. This includes but is not limited to Rochester and Kasson.

8-10 Saturdays per year; 7:00am – 12:00pm
