Description: The Case Manager RN Team Lead (RNTL) works in collaboration with clinician partners, to assist in the development of a high functioning Transition and Complex Case Management (CCM) teams. He/she practices and demonstrates skills that foster professional commitment of the team members to function to their level of licensure and training, make care decisions and assume accountability for providing high quality and timely health care which supports Kaiser Permanentes mission, vision and values. The RNTL supports the development of the Licensed Practical Nurses (LPN) and the RN Case Managers. They act as an escalation pathway for LPNs and RNs. Through the nursing process the RNTL will use critical thinking and assessment skills to guide decision making.

Essential Responsibilities:

Leadership and Professional Competencies:  

Evaluates and prioritizes, with workgroup, staffing needs daily and collaborates with manager to resolve issues.  Utilizes critical thinking to make decisions and delegate appropriate work.  
Assists with transition assessments, interventions, care plan changes and care coordination for all LPN escalations.  
Coordinates care with licensed independent practitioner at the right time to ensure patient safety.  
Serves as the escalation pathway for LPNs and CCM RNs.  
Oversees the task assignment process and practice of the LPN team.  
Collaborates with physician and clinical care teams facilitate workflow processes (e.g., help with management of things like access, referrals, waitlists, etc.).  
Advocates for patients, clinicians and staff.  
Be knowledgeable of the statutes and regulations governing LPN practice and practice within those legal boundaries and takes action leading to escalation and resolution.  
Collaborates with physician lead team members and manager to develop policies and procedures for the Transition (LPNs) and CCM programs.
Acts as mentor and coach in assisting team members with issues and concerns related to patient care and workflows.
In coordination with manager, facilitates resolution of member concerns as they relate to direct medical care issues.  
Demonstrates effective communication with patients, clinicians, and the front-line team.  
Participates in the interview process, and selection of new team members including the training and evaluating process.  
Collaborates with management to identify, develop and facilitate orientation, and training needs.  
In collaboration with management, assists in clarifying nursing scope of practice (LPN vs RN) with the team.  
Provides input into performance evaluations.  Coordinates communication flow within the team, with other departments, regionally and nationally.  
Collaborates with physician lead and manager to co-facilitate team meetings.
Communicates work team issues to manager on a regular basis.
Under direction of the manager, communicates team performance in meeting clinical and service targets.  
Demonstrates accountability for personal ethical behavior, and assignment of tasks.  Under the direction of the manager, supports and acts as a resource related to Kaiser Permanentes policies, legal considerations, and professional ethics to team.  
Adheres to and assists in monitoring teamss adherence to Health Insurance Portability and Accountability Act (HIPPA), confidentiality policies and Principles of Responsibility (POR).  
Facilitates communication with the clinician team as needed.  
Coordinates care with licensed independent practitioner at the right time to ensure patient safety.  
Uses standard documentation, information technology and care coordination tools in the electronic medical record.  
Adheres to and assists in monitoring teams adherence to Quality of Standards.  
Demonstrates and models ethical and legal behavior in all professional activities.

Clinical Competencies:  

Provides clinical expertise and professional nursing leadership to support the health care team in meeting the needs of the patient.
Understands and participates in population-based care activities which are supported by an interdisciplinary health care team.
Demonstrates personal proficiency in using computer-based information and practice management systems for decision support and provision of care.  
Demonstrates proficiency in oversite of the Licenses Practical Nurse team specified by the boards of nursing in both Washington and Oregon.

Organizational Competencies:  

Demonstrates knowledge of and facilitates national and regional strategic initiatives at the local level.  
Demonstrates knowledge of National Committee for Quality Assurance (NCQA) requirements.  
Under the direction of the manager, supports and collaborates with leadership and the Quality Department to assure all NCQA requirements are met.  
Demonstrates leadership in applying the principles and tools of the Labor-Management Partnership.  
The RNTL will assess and assign task appropriate patient responsibilities to the LPNs and CCM RNs.  
The RNTL will facilitate transition callas for members who are identified as needing RN level of support.  
RNTL provides case management support for our CCM members simultaneously with management of other team lead responsibilities.
Ability to work independently with accountability and exercise critical thinking, discretion, and professionalism at all times.  
Good organizational and time-management skills.  
Participate in on-site events, clinics, and outreach initiatives as assigned.  Other duties as assigned.

Basic Qualifications:

 

Experience

Minimum of two (2) years of experience as a Case Management RN within a healthcare system within the past five (5) years.
Will consider other experience with ability to demonstrate critical thinking and clinical skills.

 

Education

Graduate of Accredited School of nursing.
High School Diploma or General Education Development (GED) required.

 

License, Certification, Registration

Registered Nurse License (Washington) within 6 months of hire

Registered Nurse License (Oregon) within 6 months of hire

Basic Life Support required at hire

Case Manager Certificate within 12 months of hire

This job requires credentials from multiple states. Credentials from the primary work state are required before hire. Additional Credentials from the secondary work state(s) are required post hire.

 

Additional Requirements:

Previous experience in mentoring and coaching clinical staff.
Strong written and verbal communication skills.
Proficiency with Microsoft Office Suite; MS Word, Power Point, Excel.
Ability to navigate electronic health record (or gain ability within first 3 months).
Demonstrated clinical nursing skills.
Demonstrated interpersonal and customer service skills (Assessment required for external candidates).
Demonstrated communication (written and oral) skills.
Skills in problem-solving, setting priorities and using good judgment for decisions.
Ability to work independently and efficiently under pressure.
Motivational Interviewing certification
Verbal and written fluency in other languages than English and/or Proficient in Sign Language

 

Preferred Qualifications:

Three (3) years or more experience in Case Management.
One (1) year of Acute Care Hospital experience.
Bachelor of Science in Nursing or a Master of Science in Nursing.
