Description: This position represents 2 openings.

The Infusion Center LPN, as defined by The Georgia Board of Nursing, practices professional nursing by utilizing the defined organizational policies, procedures and guidelines. The Infusion LPN, under the direction of the RN, will provide direct and indirect nursing care to patients using the nursing process. The Infusion LPN will collaborate with leadership and staff to ensure the delivery of high-quality care that meets defined standards and is in compliance with clinical guidelines and processes to optimize operating excellence. The Infusion LPN will assist in managing patient flow, providing infusion services, patient education, follow-up telephone interactions, scheduling and rescheduling appointments, review and processing referrals and documenting pertinent information into the medical record.

Essential Responsibilities:

Serving as a critical member of the patient care team, the LPN renders clinical care under the direction of Physicians, Nurse Practitioners, Physician Assistants and Registered Nurse.
Rooming patients and preparing them for their infusion visit maintaining flow of the department.
Measuring, reporting and recording vital signs before, during and at the conclusion of the infusion center visit.
Performing insertion of Peripheral IV catheter insertion.
Administers intravenous medications and fluids as determined by the Georgia State Board of Nursing under the current scope of practice for IV therapy.
Prepares and administers medications which are oral, subcutaneous or intramuscular injections as ordered by a provider.
Provides comfort measures for patients and ambulation assistance as needed.
Cleaning and stocking exam rooms, med room as needed.
Uses appropriate equipment and protocols for collecting, handling, labeling and preparing for transportation to lab including proper identification, storage procedures and techniques.
Responds to basic emergent needs of patients in the infusion center.
Monitors electronic messaging and email related to patient and provider requests and inquires.
Manages the messages as appropriate or triages to physician or others as needed.
Provides telephonic and electronic (kp.org) interactions with patients that require follow-up care and/or preventative outreach.
Accurately documents patient data, actions, interventions, education and patient response according to established guidelines and policies.
Provides follow-up care to patients by providing outgoing telephone calls, follow-up interaction, coordinating referrals, scheduling follow-up appointments.
Responds effectively to member / patient concerns.
Serves as an advocate for the patient.
Facilitates transitions and coordination of care across the continuum.
Serves as a role model for nursing quality and service.
Participates in PI and Quality Improvement projects within the department.
May perform other duties as defined within the scope of practice of an LPN.

Basic Qualifications:

 

Experience

N/A

 

Education

Graduate from an accredited school of nursing as a Licensed Practical Nurse.
High School Diploma or General Education Development (GED) required.

 

License, Certification, Registration

Registered Professional Nurse License (Georgia)

Basic Life Support

 

Additional Requirements:

N/A

Preferred Qualifications:

Minimum one (1) year LPN experience preferred.
Experienced Nurse with strong background in Med/Surg and/or critical care experience preferred.
Experienced with Peripheral IV starts and phlebotomy preferred.
